Parish Nurse Survey-2016

The Health Ministry team for Middleham and St. Peter’s Parish is seeking your opinion on the health needs of our congregation and community. We are collecting this information in order to help us plan the programs and priorities for our ministry. As many of you know, the roles of a parish nurse are to function in the following capacities: integrator of faith and health, health educator, health counselor, referral advisor, health advocate, and developer of support groups.
