suicide or any attempt thereat while sane or insane; (b) Commission or attempt to commit a felony or an assault; (c) Declared or undeclared war or act of war; (d) Active duty service in the military, naval or air force of any country or international organization; (e) Voluntary ingestion of any narcotic, drug, poison, gas or fumes, unless taken as prescribed by a physician; (f) Operating any type of vehicle while under the influence of alcohol or any drug, narcotic or other intoxicant; (g) Bungee jumping; parachuting; skydiving; parasailing; hang gliding; (h) Flight in, boarding or alighting from an aircraft or any craft designed to fly above the Earth’s surface (except as a fare-paying passenger on a regularly scheduled commercial airline); (i) Services or treatment rendered by a health care professional who is employed, retained by, related to, or living with the covered person; providing homeopathic, aroma therapeutic or herbal therapeutic services; or (j) Sickness, disease, bodily or mental infirmity, bacterial or viral infection or medical or surgical treatment thereof (except for any bacterial infection resulting from an accidental external cut or wound or accidental ingestion of contaminated food).

The following is general information about the exceptions and limitations that may apply to the benefits described. This is not a complete list of policy terms and conditions. Your actual policy may vary by plan design and location. Contact your Cigna sales representative for more information. Depending on your plan, benefits may not be paid for an illness or injury that existed before the effective date of coverage. Age-based reduction of benefits and benefit waiting periods may also apply.

Accidental Injury Benefit limitations Benefits are only payable for covered injuries diagnosed and treated by a physician and resulting directly and independently from a covered accident. Under most plans, covered injuries must be diagnosed and treated by a physician within 90 days of a covered accident treatment must begin within 90 days of the accident. Physician office visit limited to one benefit per accident – excludes routine health examinations or immunizations, visits for mental or nervous disorders, or visits by a surgeon while confined to a hospital; Diagnostic exam limited to one benefit per accident and one accident per month; Dislocation/fracture if there is more than one type of fracture or dislocation, only one benefit will be paid for each injury per accident, whichever is greater – chip fracture not paid in addition to closed fracture; Burns not payable for sunburns; Cuts requiring stitches for multiple lacerations; Follow‑up physician visit and physical therapy, each benefit is limited to 10 treatments per accident and one accident per month, physician recommendation is required, all treatments must be completed within 365 days of the accident; Hospital stay payable once per accident and limited to one hospitalization per month; hospital stay and ICU stay benefits are payable once per day, subject to 365‑day and 90‑day maximums, respectively; only one hospital/ICU stay benefit is payable per covered person, per accident, whichever is the greater amount; Emergency room visits payable once per covered person, per accident and limited to one accident per month.

Critical illness Benefit limitations Specific benefit exclusions and limitations may apply to the coverages described: Skin cancer limited to 1x every 12 months with 12 months in between diagnosis; Invasive cancer excludes premalignant conditions or conditions with malignant potential, carcinoma in situ, basal cell carcinoma, squamous cell carcinoma of the skin, unless metastatic disease develops, melanoma that is diagnosed as Clark’s Level I or II or Breslow less than 0.75mm, or melanoma in situ, or may exclude prostate tumor that is classified as T-1a, b, or c, N-0 and M-0 on a TNM classification scale; also may exclude the recurrence or metastasis of an original cancer that was diagnosed before the coverage effective date if they have not been treatment free for a period of time; Carcinoma in situ excludes premalignant conditions or conditions with malignant potential, skin cancers (basal/squamous cell carcinoma or melanoma/melanoma in situ); Stroke excludes transient ischemic attacks (TIAs), brain injury related to trauma or infection, brain injury associated with hypoxia or anoxia, vascular disease affecting eye or optic nerve or ischemic disorders of the vestibular system; Coronary artery disease excludes angioplasty (percutaneous coronary intervention) and stent implantation or similar procedures; Coma excludes any state of

as an inpatient and confined to the hospital, if eligible for both benefits, only one benefit will be paid per day, whichever is greater; Hospital chronic condition admission benefits are only payable if the covered person is admitted as an inpatient due to a covered chronic condition and treatment for the covered chronic condition is provided by a specialist in that field of medicine, benefits are payable once per day, and limited to one day, benefits are not payable for treatment in an emergency room, provided on an outpatient basis, or for re-admission for the same covered injury or illness (including chronic conditions), benefits are limited to one benefit every 90 days; Hospital observation stay benefits are only payable if the covered person receives treatment for a covered injury or illness in a hospital, including an observation room, or ambulatory surgical center, for more than 23 hours following a one-hour elimination period, on a non-inpatient basis, and a charge is incurred, benefits are not payable if the covered person is admitted as an inpatient, benefits are payable per 24-hour period, and limited to 72 hours, this benefit is not payable if a benefit is payable under the hospital stay benefit or hospital ICU stay benefit.

Common exclusions Benefits may not be paid for a condition that existed before your effective date of coverage. The date of diagnosis must occur while coverage is in force and the condition definition must be satisfied. Only one initial benefit will be paid for each covered condition per person and benefits will be subject to separation periods and maximum lifetime limits. Additional benefits may be available under the recurrence benefit, if included. Benefits may not be paid for any loss that is the result of: (a) Intentionally self-inflicted injury, suicide or any attempt thereat while sane or insane; (b) Commission or attempt to commit a felony or an assault; (c) Declared or undeclared war or act of war; (d) Active duty service in the military, naval or air force of any country or international organization (Reserve or National Guard active duty training is not excluded unless it extends beyond 31 days); (e) Voluntary ingestion of any narcotic, drug, poison, gas or fumes, unless taken as prescribed by a physician; (f) Operating any type of vehicle while under the influence of alcohol or any drug, narcotic or other intoxicant; (g) A diagnosis not in accordance with generally accepted medical principles prevailing in the U.S. at the time of diagnosis.

Common exclusions Benefits will not be paid for any loss that is the result of: (a) Intentionally self-inflicted injury, suicide or any attempt thereat while sane or insane; (b) Commission or attempt to commit a felony or an assault; (c) Declared or undeclared war or act of war; (d) Active duty service in the military, naval or air force of any country or international organization (Reserve or National Guard active duty training extending beyond 31 days); (e) Voluntary ingestion of any narcotic, drug, poison, gas or fumes, unless taken as prescribed by a physician; (f) Operating any type of vehicle while under the influence of alcohol or any drug, narcotic or other intoxicant; (g) Services deemed by the insurer as not being medically necessary; (h) Elective or cosmetic surgery; (i) Dental surgery, unless due to accidental injury; (j) Services or treatment rendered by a person employed or retained by the covered person, providing homeopathic, aroma therapeutic or herbal therapeutic services, living in covered person’s household, or who is a parent, sibling, spouse or child of the covered person; (k) In certain instances, pregnancy, including childbirth, occurring within a specified period of time following the date coverage is effective.

Hospital Care (indemnity) Benefit limitations Hospital admission payable once per day, limited to one day per admission and one benefit every 90 days, must be admitted as an inpatient to the hospital, excludes treatment in an emergency room or provided on an outpatient basis, or for re-admission for the same covered injury or illness (including chronic conditions); Hospital ICU stay and hospital stay payable once per day, limited to 30 days and one benefit every 90 days, stays within 90 days for the same/related injury or illness are considered one stay, must be admitted
