Parents & Guardians DBC Missions Ministry Forms for Minors

Attached are copies of two (2) types of forms that need to be signed by the parents and/or guardians of everyone going on a Dunwoody Baptist Church Short Term Mission Trip who is younger than 18. Please follow the instructions below.

1. Dunwoody Baptist Church Mission Trip Release Form for Minor a. Sign two (2) originals and have them notarized b. Return both originals back to Dunwoody Baptist Church

2. Minor Travel Release Form a. Sign three (3) originals and have them notarized b. Return two (2) originals back to Dunwoody Baptist Church c. Keep the 3rd original; your youth/child should put it with their passport and take it on the Dunwoody Baptist Church Mission Trip

Dunwoody Baptist Church Short Term Mission Release Form for Minor We/I, the undersigned, desiring that ______________________________ (“the minor”) be allowed to visit foreign countries with Dunwoody Baptist Church and participate in a Dunwoody Baptist Church Mission Trip organized by Dunwoody Baptist Church, do hereby release and forever discharge Dunwoody Baptist Church, and their respective members, employees, officers, directors and representatives from any and all claims for any and all injuries, losses or damages the minor might have on or in any way relating to such Trip, including, without limitation, those relating to the minor leaving the United States of America and visiting foreign countries, including the minor’s stay in any such foreign country and the minor’s trip to and from any such country. We are/I am eighteen (18) years of age or older, and this Release is binding on the undersigned and the executors, administrators, and heirs of the undersigned. The undersigned give Dunwoody Baptist Church and its representative(s) with the minor on any such trip authority to request and authorize medical and/or hospital treatment for the benefit of the minor in the event of any injury or sickness sustained by the minor while on such Dunwoody Baptist Church Mission Trip, including, all such treatment and to reimburse Dunwoody Baptist Church for all costs and expenses incurred by it with respect to such treatment.
