Please initial in the blank next to each statement. I do hereby agree to the following: ______ To provide JOY School with all required forms at time of enrollment and to provide updated immunization records throughout the year. ______ To pay monthly tuition on the first day of each month. (Late fees of $10 will be applied on the 5th, 10th, 15th, 20th, and 25th of the month until payment is received. If tuition becomes one month delinquent, the student will be dismissed from the school.) ______ That if I become delinquent in payment of fees and/or tuition, JOY School may take appropriate actions in dealing with delinquent accounts. These actions may include, but are not limited to, efforts to collect any past due amounts. Failure of JOY School to exercise any rights hereunder shall not be considered or deemed a waiver of any such rights. ______ To notify the school in writing if I choose to no longer have our child attend JOY School. A one month’s written notification or one month’s tuition payment is required for withdrawal. ______ That JOY School or Glen Meadows Baptist Church reserves the right to use students pictures in our brochures, newsletters, newspaper articles and clips, billboards, school publications, school website and other marketing instruments unless otherwise indicated by parent. ______ To have my child picked up on time. If I fail to do so, I understand that I will be charged a $5.00 initial late pick-up fee and $1.00 for every minute late after the appropriate pick-up time. ______ To have my child picked up within 30 minutes of receiving a phone call from the school regarding sickness or behavior.
