Dear Parents/Guardians, Please find below the New Jersey State Department of Health & Senior Services immunization requirements for entrance into Kindergarten (N.J.A.C. 8:57-4). 1) DTaP (Diphtheria and Tetanus Toxoids, and Pertussis) - 4 doses with one dose given on or after the 4th birthday, or any 5 doses. 2) Polio- 3 doses with one dose given on or after the 4th birthday, or any 4 doses. 3) MMR (Measles, Mumps, Rubella)- 2 doses. 4) Varicella – 1 dose. 5) Hepatitis B- 3 doses. If your child has been immunized with the above requirements and you have not provided documentation to the school nurse, please do so as soon as possible. If your child will be vaccinated at your child’s next well visit please provide the documentation before School starts in September to prevent exclusion. If you are uninsured contact your local health department to inquire about free immunizations. May God bless you and your family as your child begins Kindergarten.
