Flex Promise Program

Once enrolled, participating in the Flex Promise Program is as easy as 1, 2, 3:

Our Flex Promise Program states that if any patient does not use their prescribed Philips Respironics continuous positive airway pressure (“CPAP”) device with “Flex” technology with the regularity required by the patient’s insurer/third party payor in order for the patient to receive reimbursement (i.e. is Noncompliant), then Philips Respironics will provide purchaser a credit towards the prior purchase of a Philips Respironics bi-level positive airway pressure (BiPAP) device with “Bi-Flex” technology equal to the purchase price of the patient’s CPAP device with “Flex” technology less a $95.00 handling fee. Some restrictions apply.

After purchasing a Philips Respironics BiPAP device* with “Bi-Flex” technology as a replacement for a CPAP device** with “Flex” technology: Complete and sign the Flex Promise Program Device Replacement Request Form (Form 4420).

In addition to being able to minimize used inventory and to alleviate some of the burdens associated with transitioning a patient to a more advanced device, you can:

Return the Form 4420 via email to [email protected] or fax it to 855-500-4202 (USA) or 888-548-4714 (Canada). We’ll reply with return authorization (RA) and shipping labels.

• Meet the changing needs of your patients without carrying the cost of more than one device • Remove barriers that might prohibit a patient’s move to a more clinically appropriate device • Partner with your patients and their physicians to provide a BiPAP rescue option for struggling patients.

Return the device.

Note: This Program is not to be used for devices with quality defects. For reports of devices with quality defects, contact Customer Service at 1-800-345-6443. The credit of the returned device may only be used to purchase a new BiPAP device. Certain other terms and conditions apply.

*BiPAP devices include BiPAP Pro, BiPAP Auto, BiPAP autoSV, BiPAP AVAPS, BiPAP S/T. **Does not include DreamStation Go
